There are two routes to creating a video self reflection or video observation form, depending on whether you're still on the video upload page, or if the video has been uploaded earlier (or by another staff member). If you're yet to upload your video file, you must start there—head to this help doc to find out how.

Create a video form in the video library
If you've just uploaded a video, you should be able to start a form about it as soon as the main upload process is complete—even while the video is still processing!
- Locate the video file you've just uploaded and then click the Forms button at the bottom of the video card.
- Select New Form to open a window to create the form.
- You can 'schedule' the form for a future date, or choose today's date to start the form right away.
- [Optional] You can add text to the Grade, Subject, and Location fields and choose to send an email notification to the topic of the video, which will let them know that a form has been created (along with the title of the form, date, and any other details entered here).
Create a video form from your dashboard
If another staff member has already uploaded a video file for you to start a form, then you can also start a video form from your dashboard:
- Click the New Form button to open up the form creation window
- Select the staff member who you are creating a form about in Who is this form about? then hit Next
- Locate and select the type of form you want to create (pro tip: video templates have a video type icon to the left of the name) then click Next
- You can 'schedule' the form for a future date, or choose today's date to start the form right away.
- Select the video file that you want to associate with the form in the Video dropdown.
- [Optional] You can add text to the Grade, Subject, and Location fields and choose to send an email notification to the topic of the video, which will let them know that a form has been created (along with the title of the form, date, and any other details entered here).
Note: If you don't see the video file you need and you know it's already been uploaded, try locating the video from the library, where it's easy to search for the video you need by person and date—find out how here!
